In order for Cyclebrakes, Inc. to maintain a valid list of business establishments who are qualified wholesale purchasers of parts, we need certain information. Cyclebrakes, Inc. reserves the right to decline doing business with anyone who does not meet the following qualifications: the business must operate on a full-time basis in an established, permanent and commercial location. Approval of dealers shall be within the sole discretion of Cyclebrakes, Inc. and may be withdrawn at any time, with our without cause.
This form must be filled out completely and a copy of a current business license must be attached before it is processed.
